On the Ohio State University campus, he was arrested for three separate instances of rape, after his conviction for a number of offences, one of which was armed robbery. During the process of preparing his defence, Milligan was diagnosed with dissociative identity disorder by the psychologists working on the case. His attorneys filed a plea of not guilty by reason of insanity, arguing that the acts were perpetrated by two of Milligan’s alternative personas without his knowledge. He was the first person who had been diagnosed with dissociative identity disorder to raise such a defence, and he was also the first person to be cleared of a serious crime for this reason. He spent the next ten years in mental facilities rather than being convicted of the crime.
The Minds of Billy Milligan, an award-winning non-fiction book written by Daniel Keyes, is largely responsible for the widespread dissemination of Milligan’s life narrative.

On December 12, 2014, Milligan passed away from cancer while residing in a nursing facility in Columbus, Ohio. He was 59.
